This document contains important information about my professional services and private practice business policies. Please read it carefully. When you sign the consent form for this document, it will represent an agreement between us. 

Be sure to schedule time before our first appointment to open and review the HIPAA Notice, the Informed Consent, and the Teleconsent documents, which can be accessed using the buttons below.  At our first appointment I will answer any questions you may have about these documents and then ask you to electronically sign consent agreements regarding them

This notice describes how health information about you may be used and disclosed and how you can get access to this information.  Please review this notice carefully.

Please use the "New Client Form" button to open up the form that requests information about you and your goals for career/life counseling/coaching.  Be sure to fill out the two-page form completely.  Every item of information requested is important to our work.  Once you have completed the form, save it ("save as") as a PDF in a place on your device that is easily accessible.  During our first virtual appointment, I will ask you to submit the form through a secure electronic file share.

Payment


You will be able to pay for each session through the telehealth platform with a credit or debit card.

How a Telehealth Session Works

Telehealth is the practice of delivering clinical counseling or other health care services via technology assisted media or other electronic means between a practitioner and a client who are located in two different locations.  We will be using HIPAA-compliant video technology, which means that our appointments will be encrypted and secure. The audio/video record will be deleted automatically when our appointment is over.

To participate in a telehealth session with me, Elizabeth S. Arnold, LPC, located in Austin, Texas, you can use any device that has a camera, a microphone, and connectivity to the Internet. You will put this link to my virtual office into your browser or click on it here:
www.doxy.me/BethALPC. When you click on the link you will be directed to my virtual waiting room. I will click on your name when I see you have “arrived”. After a short pause we will be connected and ready to start our session.

During your first telehealth appointment, we will begin by discussing the client information you provided and any questions you may have about the other documents on this page.  Then I will ask you to electronically sign the teleconsent form and the two other consent forms related to HIPAA Practices and Informed Consent. After that we will be free to begin our work.
